Abstract

Uganda’s adolescent population (ages 10–19) is experiencing a "Silent Emergency"—a rapid, systemic escalation of mental health disorders that remains largely invisible in national health and education discourse. Following the world’s longest nationwide COVID-19 school closure (22 months), the psychological landscape has shifted from localized stress to a pervasive crisis. This desk review evaluates the prevalence, drivers, and systemic barriers to mental healthcare among Ugandan adolescents between 2021 and 2026, focusing on the disconnect between national policy and grassroots implementation. A qualitative desk review methodology was utilized, synthesizing 30 recent sources, including peer-reviewed journals, UN/NGO reports, and policy documents. Data were thematically coded across four pillars: prevalence, etiology, cultural-systemic barriers, and policy-implementation gaps. Findings reveal alarming disparities: out-of-school adolescents in urban settings like Kampala exhibit significantly higher depression rates (21.5%) compared to their in-school peers (14.0%). Key drivers include the "lockdown lag," academic pressure, poverty, and gender-based violence. Despite the existence of the Ministry of Health’s Child and Adolescent Mental Health (CAMH) Policy Guidelines, implementation remains fragmented due to chronic underfunding, severe shortages of trained professionals, and deep-rooted cultural stigma. To address this silent emergency, the study recommends the urgent integration of mental health literacy into Uganda’s Competency-Based Curriculum (CBC), the decentralization of care through school-linked task-sharing models, and treating adolescent mental health as a strict cross-sectoral priority.
